Chronic Care Management – CCM/PCM/APCM
At RMS, our team believes in holistic, whole person care. Our team understands that chronic care management just for the purpose of collecting minutes for billing is not the proper way to impact change. We are dedicated to truly creating a positive impact in the life of the patients, caregivers and medical providers we work with. In each community we serve, we research and build a comprehensive list of community resources that our care coordinators can utilize to real-time assist your patient with their community resource needs, thus improving patient satisfaction and alleviating practice burden. Additionally, our team believes in addressing the spiritual needs of the patients as well, which is something not done within most CCM programs. We have several ways in which we can serve your practice:

- We can create and maintain care plans, educate patients and caregivers on disease specific topics, submit questions and concerns identified to the practice and then follow up with the patient/caregiver you have provided feedback, and we will work with the patient/caregiver to help facilitate SDOH community resource-based needs.
