Tracy M. Polak, MSN, CRNP, NHA, is a distinguished clinical leader and educator with over 25 years of experience in post-acute and long-term care, primarily in skilled nursing facilities.
Her passion lies in optimizing the health and well-being of frail older adults in post-acute and long-term care settings. Tracy advocates for an interdisciplinary team approach, ensuring that older adults receive the right care at the right time in the right place. Throughout her career, she has developed several community-based and skilled nursing facility clinical models of care, enhancing the lives of older adults, their families, and the staff who care for them.
Tracy’s extensive clinical, administrative, and health plan experience enables her to drive clinical, quality, and financial outcomes by enhancing care delivery and supporting staff, providers, patients, and their families.
Tracy is an adjunct faculty member at the University of Pittsburgh School of Nursing. She is actively involved in professional organizations such as The Pennsylvania Society for Post-Acute and Long-Term Care Medicine (PMDA).
